The item acts as a normal Pipe of Smoke Monsters except when the words have been read out loud. While smoking this pipe and the engraved words have been read out loud, the letters faintly glow in green color. You can then use an action to exhale a puff of smoke that takes the form of a naked halfling male that looks like Rudi, flexing his muscles, instead of any single creature such as a dragon, a flumph, or a forghemoth.
