Character Caecus Drayven was killed and turned to dust by a Doomguard. A planescape 'glitch' occured and a multiversal version of my character appeared. The character can be rebuilt in any way, including a different name, but there will be a common physicality between Caecus and my current character to identify them as 'glitch copies' (think Loki or Dr. Strange Multiverse character.) [Character was rebuilt from half-elf to variant human]
